Appointment Information

Please bring comfortable clothing to wear. This may include a pair of shorts and a tank top or a swim suit. The hands-on portion of treatment is most effective on skin. No oils or lotions are used.

Insurance Information

CHRYSALIS PHYSICAL THERAPY IS CURRENTLY NOT ACCEPTING NEW MEDICARE PATIENTS. Unfortunately new medicare appointments are no longer being offered.

Chrysalis Physical Therapy does not participate in-network with any insurance company. Payment for all sessions is due at the time of the appointment. We will happily provide you with a claim form that you can submit to your insurance company for reimbursement.

Prescriptions

New York state law allows patients to receive physical therapy treatments for either 10 visits or 30 days without a doctor's prescription. After this time/visit frame, a prescription is required by law to continue treatment
Please note that some insurance companies require a prescription from the first visit, in order to justify reimbursement
